Graduation Requirements

Graduating students will receive their hard-earned diplomas after completing a set of academic, financial and administrative requirements. The PUMS diploma will become your passport to your international professional career in the health sciences. To celebrate the graduating class’ success, the University holds annual graduation ceremonies in May/June.

Requirements for receiving diplomas

I.Academic — full completion of the program’s curriculum.

II.Financial — complete payment of all school fees, including dormitory rent.

III. Administrative — submitting the following documents to the Dean’s office:

1) Graduation documents:

• Personal data survey – should be identical to the data in your passport. If you have two passports, please specify which one was used.
• Authorization form (required only if you cannot pick up the diploma in person).

2) Complete set of completed examination cards, and, for medical students, also all elective evaluation forms,

3) Completed practical summer training booklet,

4) Completed practical skills record booklet – medical students (may be submitted upon collection of diploma),

5) Student Clearance Form – completed online in VISUS -> AKSON -> ASIA: choose your year of study, click on the word “obiegówka” (student clearance form) and then choose “stwórz obiegówkę” (generate the clearance form). Your status with the dormitories, library, study rooms and bursary office will subsequently be verified for any outstanding payments or irregularities. This requirement will be considered completed once all of the above departments confirm your ‘cleared’ status,

6) Please check your personal data in our student system: first name, middle name, last name, date and place of birth, as this data will be used to generate your diploma document.

The Dean’s Office will issue the hard copy of a diploma within 30 days from the day of fulfillment of all the above requirements and receipt of the complete set of documents. Please notify your class coordinator at the Dean’s Office once you complete all of the requirements above.
